The employee’s supervisor needs to call and report the claim to GB Services at (800) 780-9561. It is recommended that the 1^st Report of Injury be completed before calling GB Services. The 1^st Report of Injury can be found on the Insurance office’s web page under Insurance/Forms and Documents. Much of the information that GB Services will ask for is on this report. GB Services will give you a report number and that number should be written down on your 1^st Report of Injury. Keep this report in your files for future reference.
